Chatham Oaks, Inc.

Chatham Oaks provides two main services - Residential Care Facility (RCF) and Home-Based Habilitation services. The Residential Care Facility provides services 24 / 7 for adults with a mental illness. The focus of services is to provide support for the individual to increase skills for independent living. Services include nursing, recreation and skills training. Each resident is assigned a social worker. Home-Based Habilitation is a Medicaid funded service. Services are provided to individuals with a mental illness who need support while living independently in their own home or apartment.

Mobile Crisis Outreach Program

Call 1-855-581-8111 and ask to have Mobile Crisis Outreach dispatched in Johnson and Iowa Counties.

With Mobile Crisis Outreach, mental health counselors are dispatched to homes, schools, businesses, or public spaces where a mental health crisis is occurring. Response time is within 60 minutes of dispatch.

CommUnity Crisis Services and Food Bank

Residents of Johnson County, IA can receive grocery assistance once per week. Clients may choose their own items, with some limits based on family size. Pre-made bags are available for clients who prefer not to shop.

First-Time Visit Information:

Please arrive at least 30 minutes prior to closing so we can gather family information in a 15-20 minute first-time client interview.
