Description Of Organization And Services

The focus of the City of North Liberty Transportation Assistance Program (NLTAP) is to assist eligible residents over 18 years old who may - for various reasons - be unable to travel to important destinations in and near North Liberty for essential quality-of-life services. After the application has been received by staff, a home visit appointment will be arranged to introduce the applicant to the program, procure the image for the NLTAP identification card, and discuss other area resources & programs.

To apply, fill out the online form OR print your own (PDF) and mail to NLTAP, P.O. Box 77, North Liberty, IA 52317. Printed forms are available upon request by leaving a message with your name and address at the 24/7 Transit Voicemail Inbox (319) 626-5918 or by emailing [email protected].
